   3. Check 'New Activation', if it is a new account
   4. Check how many months prepaid service and initial
   5. Check the 'type' of service
   6. Check 'options' desired
   7. Fill in Credit Card information
   8. Sign
2. Total Price is calculated:  Product Price + Service Price + Activation Fee ($19.95)
3. Fax the completed form to 810-795-9320, attn: Order Entry.  Once the service is activated the device will be sent out.
